Endovascular Removal of Obstruction

Effective October 1, 2006, a new code (39.74) has been created to uniquely identify endovascular mechanical thrombectomy of precerebral and cerebral vessels. Endovascular mechanical thrombectomy is an innovative procedure for the treatment of ischemic stroke. Ischemic strokes account for approximately 80% to 90% of all strokes and are caused by obstruction/occlusion from thrombus or embolus in the precerebral and cerebral vessels. Endovascular mechanical thrombectomy can restore patency in the neurovasculature by removing occlusion(s) and may be used for patients who are not eligible for thrombolytic therapy. Conventional treatment involved intravenous infusion of anticoagulants or thrombolytic agents, such as...

Article Overview

This article covers a coding update effective October 1, 2006 for endovascular mechanical thrombectomy in precerebral and cerebral vessels. It explains the clinical context of ischemic stroke treatment and outlines related reporting notes, associated procedure coding references, and excluded categories. It is intended for coding professionals, revenue cycle staff, and others who need to understand how this procedure topic is represented in the code set.

Why This Topic Matters

The article helps readers identify a newly created procedure code and understand the surrounding coding context for stroke-related endovascular intervention. It is useful for accurate procedure classification, code cross-referencing, and avoiding confusion with related but distinct vascular procedures.

Article Sections

  1. Overview of endovascular thrombectomy for ischemic stroke

Introduces the clinical setting for the procedure and describes the general treatment area within stroke care. It also notes the broader context in which the coding update appears.

  2. New code 39.74

Presents the new procedure code and the general category of service it represents. The section also includes related reporting notes and excluded categories.
