AHA Coding Clinic® for ICD-9 - 2009 Issue 4; Volume 3 New/Revised Codes
Cardiac Contractility Modulation
Effective October 1, 2009, a new subcategory has been created to describe the implantation of a rechargeable cardiac contractility modulation (CCM) system. This new code set is for use to report the implantation of a total CCM system (17.51) and the placement or replacement of the pulse generator (17.52). The CCM system is a new treatment modality for patients with moderate to severe heart failure resulting from either ischemic or nonischemic cardiomyopathy. In cardiac contractility modulation, nonstimulatory impulses are delivered during the absolute refractory period (ARP) in order to enhance cardiac function and contractility. The CCM system...

Article Overview
This article covers a newly created procedure code subcategory for cardiac contractility modulation systems, the components and clinical context of the technology, and related coding considerations when CCM is performed alone or with other cardiovascular procedures. It is aimed at coders, billers, and clinical coding professionals working with cardiovascular surgery and heart failure cases, especially when distinguishing total-system implantation from pulse-generator-only work and identifying accompanying diagnoses.
Why This Topic Matters
Accurate reporting of CCM-related procedures depends on recognizing the new code structure, the device components involved, and the relationship to other cardiovascular procedures that may occur in the same encounter. The article also places the procedure in a diagnostic context that can affect claim coding and documentation review.
Article Sections
-
Overview of cardiac contractility modulation
Introduces cardiac contractility modulation as a treatment modality and summarizes the general clinical setting in which it is used. Also describes the major device components at a high level.
-
Device system components and implantation setup
Describes the main parts of the CCM system and how the system is positioned and connected during implantation. The section also outlines the roles of the programmer, charger, and related hardware.
-
New subcategory 17.5 Additional cardiovascular procedures
Presents the newly created procedure code subcategory and frames the two CCM procedure codes discussed in the article. This section includes the surrounding cardiovascular procedure context referenced by the source.
-
Clinical case and coding question
Provides a heart failure case scenario and asks for the appropriate diagnosis and procedure coding approach. The discussion centers on how the procedure and diagnosis information relate to the case.
