Intravascular Imaging

Effective October 1, 2004, unique codes have been created for intravascular imaging procedures. Intravascular imaging is a catheter-based ultrasound imaging technique that allows viewing of the inside of a vessel. This technology is used in the diagnosis and management of coronary and peripheral vascular disease, as well as to visualize intracardiac structures during electrophysiology procedures. These codes should not be applied to intravascular MRI. Although both technologies “image” the vessel, IVUS is performed in real time and uses sound waves to view the lumen. MRI or magnetic resonance imaging takes pictures for delayed review...

Article Overview

This coding update covers the introduction of intravascular imaging procedure coding and how the new categories are organized by vessel type. It is relevant for coding professionals working with vascular, cardiovascular, and electrophysiology-related imaging services, and it notes the broader context of ICD-9-CM coordination and tabular updates.

Why This Topic Matters

The article helps readers recognize that intravascular imaging was given distinct procedure codes and understand the overall scope of the new vessel-based structure. It also highlights that the update supersedes earlier published guidance, which may affect historical coding references.

Article Sections

  1. Overview

Introduces intravascular imaging and the general clinical contexts in which it is used. It also notes the timing of the coding update and the relationship to related imaging methods.

  2. New subcategory 00.2 Intravascular imaging of blood vessels

Presents the new procedure category and its general framing within the classification system. It includes broad notes on the scope of the subcategory and related exclusions.

  3. New code 00.21 Intravascular imaging of extracranial cerebral vessels

Covers the vessel-specific category for extracranial cerebral imaging. It includes the general site focus and associated exclusion note.

  4. New code 00.22 Intravascular imaging of intrathoracic vessels

Covers the vessel-specific category for intrathoracic imaging. It includes the general site focus and associated exclusion note.

  5. New code 00.23 Intravascular imaging of peripheral vessels

Covers the vessel-specific category for peripheral imaging. It includes the general site focus and associated exclusion note.

  6. New code 00.24 Intravascular imaging of coronary vessels

Covers the vessel-specific category for coronary imaging. It includes the general site focus and associated exclusion note.

  7. New code 00.25 Intravascular imaging of renal vessels

Covers the vessel-specific category for renal imaging. It includes the general site focus and associated exclusion note.

  8. New code 00.28 Intravascular imaging, other specified vessel(s)

Identifies a category for cases involving other specified vessel locations. The section is brief and organizational in nature.

  9. New code 00.29 Intravascular imaging, unspecified vessel(s)

Identifies a category for cases where the vessel site is not specified. The section is brief and organizational in nature.
