Find-A-Code has made it easier to find the HCC Codes you are looking for, helping you code with more efficiency. Chronic conditions and HCC codes being used in part to determine reimbursement. If you are coding or auditing HCC Codes, your tools are now all on one page. With the first Risk Calculator on the market, use the calculator to generate the Risk Scores using CMS Risk Model with several versions available.   

What are HCC Codes? Data for Risk –Adjustments are pulled from diagnosis codes that are converted to an HCC Code that carries a risk score.
