AMA Comment No, these codes are intended to be reported by nonphysician health care professionals. Physicians would report these services with the evaluation and management (E/M) home visit codes 99341-99350. However, heath care professionals who are authorized to report codes 99341-99350 may report codes from the 99500-99600 series if both services are performed. Modifier 25 should be appended to the E/M code, if the patient's condition requires a significant separately...
